Job summary

UnitedHealth Group’s Optum unit seeks a Behavioral Health Advocate in Manhattan to coordinate care for Medicaid enrollees through field visits and telephonic work from home. The schedule is Monday–Friday 8 am–5 pm with residency in Manhattan required.

Candidates must hold a Master’s or Doctoral degree in a behavioral health field and an active NY license, plus 2+ years post-licensure. Proficiency in MS Office and reliable transportation are essential.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or related behavioral health field
• Active, unrestricted NY license (LMHC/LMFT/LCSW/LP)
• 2+ years post-licensure experience in a mental health setting
• Proficient with Microsoft Office including Word, Outlook, Excel, and Teams
• Reliable home office setup with high-speed internet and ability to travel daily in Manhattan area

Responsibilities

• Identify and provide community service linkage
• Assist enrollees with aftercare appointments
• Provide follow-up as determined by member needs
• Identify gaps in care and problem-solve to connect to services
• Review IP Census to track re-admits in last 12 months
• Attend facility meetings to develop aftercare plans
• Oversee aftercare follow-up for high-risk enrollees
• Meet with providers to foster rapport and identify new programs
