Job summary

EPIC Health in Houston, TX is seeking a qualified Advanced Practice Provider (NP/PA) to join our outpatient primary care team. The role emphasizes prevention, chronic disease management, and patient-centered care with a focus on Mandarin or Spanish speaking patients.

You will collaborate with physicians, care managers, behavioral health specialists, and a multidisciplinary team to deliver comprehensive care, conduct wellness visits, order tests, educate patients, and promote continuity of care

Qualifications

• Master’s or Doctorate in NP/PA program from an accredited institution.
• Active Texas license as NP (FNP) or PA-C.
• Active DEA license or ability to obtain prior to start.
• National certification applicable to role.
• Strong commitment to patient-centered and culturally competent care.

Responsibilities

• Evaluate, diagnose, and treat patients in outpatient primary care setting.
• Conduct wellness visits, preventative screenings, chronic disease management, and acute care visits.
• Develop and implement individualized treatment plans.
• Order and interpret diagnostic tests, labs, and imaging.
• Educate patients on medications, treatment plans, nutrition, and preventative health.
• Support patients in managing chronic conditions such as diabetes, hypertension, and asthma.
• Promote continuity of care through proactive follow-up and patient engagement.
• Collaborate with physicians and interdisciplinary care teams.
• Coordinate referrals, diagnostics, and specialty follow-up care.
• Participate in a team-based, integrated care model focused on quality outcomes.
