Job Description

The Manager of Care Management (RN or MSW) at our Medical Center in Fullerton, CA, is a full-time position responsible for leading and managing the Care Management Team, which includes RNs and Social Workers.

Under the direction of the Director, Care Management, the Manager oversees the daily operations and clinical practices within the assigned service areas. This role involves supervising Case Managers and Care Management Coordinators to ensure proper case management and advocating for patients to achieve optimal health, access to care, and resource utilization.

Required Qualifications

• Bachelor’s degree in nursing or master’s degree in social work
• At least 3 years of experience in an ambulatory or acute care setting
• Strong project management and operations skills

Preferred Qualifications

• Master’s degree in nursing or health administration
• Case Management Certification (CCM or ACM) upon hire
• At least 3 years of management experience in a healthcare setting

Ideal Candidate

• Holds a California RN license or California Social Work license
• Minimum 3 years of experience as an RN or Social Worker in a hospital or inpatient setting
• Minimum 1 year of leadership and people management experience
