Job summary

Penn State Health in Camp Hill, PA is seeking a Part Time, 0.50 FTE RN to manage stroke transition from inpatient to outpatient care. You will work with Stroke Program coordinators, clinical providers, care coordinators, social workers, and clinic staff to ensure smooth patient handoffs and continuity of care.

Required are a BSN, at least 3 years of nursing experience, and AHA BLS before orientation ends, along with an active RN license in the employing state or a multi-state license.

Qualifications

• Registered nurse with a bachelor's degree required.
• Minimum three years of nursing experience.
• AHA BLS prior to end of orientation period.
• Currently licensed to practice as a Registered Nurse in the state of employment or holds a multistate RN license.

SUMMARY OF POSITION:

Responsible for stroke transition from inpatient to outpatient care. Works with Stroke Program coordinators, clinical providers, care coordinators, social workers, and clinic staff. Collaborates with Community Paramedicine team and Clinical RX-2-Go Pharmacist.

MINIMUM QUALIFICATION(S):

• Registered nurse with a bachelor’s degree required.
• Minimum three (3) years of experience in nursing required.
• AHA BLS prior to end of orientation period.
• Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.

PREFERRED QUALIFICATION(S):

• Preferred nursing experience in neuroscience, emergency department, or quality
• Stroke Certification (Examples include: ASC, NVRN, SCRN, CNRN)
