Job summary

BMC Berkeley Medical Center in Martinsburg, WV is seeking a Patient Safety Leader to guide the hospital's safety program across inpatient and outpatient settings. You will analyze data, lead initiatives, and coordinate a multidisciplinary team to reduce errors and improve outcomes.

The role directs the Patient Safety Committee, conducts investigations (RCA/ FMEA), and drives education, training, and culture of safety throughout the organization.

Qualifications

  • Bachelor’s degree in healthcare or related field.
  • Five (5) years in patient safety setting.
  • Master’s degree in healthcare or related field (preferred).

Responsibilities

  • Promotes and supports a culture of safety by planning, managing and coordinating activities at the hospital and medical practices related to patient safety.
  • Identifies, collects, organizes, analyzes, and reports patient safety data and trends and develops recommendations to support continuous improvement initiatives.
  • Develop and lead the Patient Safety Committee, facilitates actionable outcomes, and serves on relevant hospital and system committees, councils, and task forces related to patient safety and quality improvement.
  • Serves as a conduit to all levels of hospital staff by providing education, consultation, updates, reports, and communication regarding patient safety initiatives, concerns, trends, and opportunities for improvement.
  • Serves as a facilitator for the implementation and sustainment of a Culture of Safety throughout the organization through education, training, staff engagement, and the promotion of evidence‑based patient safety practices.
  • Formulates strategies to identify, communicate, and mitigate patient safety risks and vulnerabilities and identifies opportunities for proactive improvement based on internal trends, external events, industry best practices, and published recommendations.
  • Facilitates patient safety investigations, such as Root Cause Analyses (RCA), Apparent Cause Analyses (ACA), Common Cause Analyses (CCA), Failure Mode and Effects Analyses (FMEA), significant events, errors, and near misses; ensures timely completion of investigations, implementation of corrective actions, and ongoing evaluation of improvement strategies to prevent recurrence.
  • Reviews patient safety events and provides guidance regarding event classification, escalation, investigation, resolution, and adherence to organizational patient safety practices and standards of care.
  • Develops, coordinates, and supports patient safety education, orientation, competency, and training programs for healthcare providers, staff, and leadership.
  • Serves as a liaison with other healthcare organizations and external groups focused on patient safety improvement and represents the organization in collaborative patient safety initiatives when appropriate.
  • Selects, hires, manages, evaluates, mentors, and facilitates the professional development of staff and provides leadership, training, mentoring, and oversight to Patient Safety Coordinators, including guidance related to serious safety events, complex investigations, and escalated cases.
  • Provides leadership for continuous process improvement initiatives related to patient safety operations, workflows, team activities, and organizational patient safety goals.
  • Develops, prepares, and presents patient safety reports, analyses, findings, recommendations, and educational updates to leadership, committees, physicians, faculty, and staff.
  • Responsible for completing complex cases and tasks escalated by associate‑level staff, ensuring timely and accurate resolution.

Job description

Provide leadership of patient safety functions to ensure a safe environment for patients, families, and employees. Provides expertise and leadership through critical analysis, assessment, implementation, and evaluation of clinical care in inpatient and outpatient settings. Accountable for the planning, coordination, implementation, and evaluation of patient safety programs and initiatives. Uses a comprehensive view to identify vulnerabilities within the healthcare system and implement changes that result in error reduction and prevention. Oversees the Patient Safety Committee and is responsible for safety event classification and/or adjudication when needed, as well as the initial and ongoing training, mentoring, and oversight of the Patient Safety team. Identifies, collects, organizes, analyzes, and reports patient safety data and facilitates development of solutions and recommendations that promote safe practices and positive outcomes. Responsible for completing complex cases and tasks escalated by associate‑level staff, ensuring timely and accurate resolution, while managing and coordinating patient safety activities across the organization. Requires collaboration across multiple teams and regional hospitals and ambulatory sites.

MINIMUM QUALIFICATIONS:
