Meridian Healthcare seeks an experienced FACT Manager to lead a community-based care team integrating behavioral health, medical services, and social supports. You will guide staff, promote client-centered care, and work to reduce barriers to care.
Requires a Master's degree in health/psychology/social work, Florida license (LCSW/LMFT/LMHC), 3 years in mental health management, 5 years with Serious Mental Illness or co-occurring disorders, and a valid FL driver's license.
