Description:

Hospital at Home (HaH) is an innovative program designed to deliver inpatient-level healthcare to patients in the comfort of their homes. By extending hospital-level care outside the traditional hospital setting, we are redefining healthcare delivery. This program ensures patients receive necessary medical treatments while maintaining the comfort and familiarity of their own homes, thereby improving patient satisfaction and reducing hospital readmissions.

In this role, you will:

  • Maintain a liaison relationship with the HaH department through participation in patient care rounds and joint projects, including but not limited to:
  • Identifying problems/issues in specific patients and promoting rational drug use (therapy selection, monitoring, and assessment of outcomes)
  • Providing drug information to providers covering the service and patients in the HaH program
  • Reporting adverse drug reactions for patients on service
  • Verifying medication orders and coordinating medication dispensing
  • Conducting and overseeing medication reconciliation upon patient admission to their homes

Participate in continuous quality improvement and expense reduction efforts related to patient care. Outcomes of these efforts should include the development of:

  • Drug use evaluations
  • Treatment protocols and guidelines
  • Care maps and critical pathways
  • Other related products/services

Preferred Criteria:

  • Completion of a general or specialty residency or fellowship is preferred
  • One to three years of clinical management experience is preferred

Minimum Requirements:

  • Completion of an entry-level or post-baccalaureate Doctor of Pharmacy program
  • New York State licensure or eligibility for licensure as a Pharmacist
  • Knowledge of pharmacotherapy, interpersonal communication, and computer applications

Salary Range:

$152,000-$199,200 annually
