Job summary

Providence Regional Medical Center Everett is seeking a Manager of Care Management to lead the department and ensure high-quality, coordinated patient care. You will oversee care coordination, discharge planning, utilization review, social work services, and pre-admission management within a comprehensive hospital setting.

The role requires an advanced Social Work degree, a state social work license, and several years of case management with leadership experience in a large hospital.

Qualifications

• Master's Degree in Social Work is required.
• Active Washington Clinical Independent Social Worker license required.
• Minimum 6 years of experience in Case Management in acute care.
• Minimum 2 years of managerial/director level experience in a large hospital.

Responsibilities

• Oversee daily management and organization of the Care Management Department.

The Manager Care Management is responsible for the daily management and organization of all activities and staff within the Care Management Department including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management. The Manager acts as a liaison between patients and families, department staff, physicians and the healthcare team, community agencies, managed care representatives, skilled nursing facilities, and community agencies.
