  • 6 years of experience in Case Management.
  • 2 years of Supervisory/management experience, preferably in Case Management.
  • Previous nursing experience within a variety of complex client care areas.
  • Experience in problem-solving and conflict resolution.
  • Experience in providing adult education to multi-disciplinary staff.

Responsibilities

  • Manage daily operations of the Case Management Department and supervise staff.
  • Coordinate care, discharge planning, and utilization management.
  • Serve as liaison among patients, families, staff, physicians and community partners.

The Manager is responsible for establishing, implementing and monitoring best practice and established clinical standards to assure the quality of patient care and the financial integrity of the organization.

Required Qualifications

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
