Position Overview

Managed Care Nurse Practitioner, Adult and/or Gerontology
PRN Position Available
PHP Nurse Practitioners are dedicated to delivering quality care to seniors and residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living, and via telehealth.

We are currently recruiting a PRN Managed Care Nurse Practitioner passionate about providing high-quality care. This position is based at Worthington Healthcare Center in Parkersburg, WV, and Belmont Healthcare Center in Belmont, WV. Candidates must be available at least 2 days per week.

Our Nurse Practitioners work Monday through Friday, first shift. No on-call, weekends, or holiday coverage.

Job Summary

The PHP NP provides high-quality primary care within a person-centered care model, diagnosing, treating, and managing acute and chronic illnesses through wellness visits and early detection. The NP collaborates with PCPs and interdisciplinary teams, involving residents and families in care planning, reducing hospital and ER visits, and fostering a culture of customer satisfaction.

Responsibilities

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret tests, adjust treatments accordingly.
• Maintain compliant documentation in EMR.
• Collaborate with leadership and support strategic initiatives.
• Ensure excellent customer experience.
• Practice ethically per standards and regulations.
• Coordinate care with providers and specialists, including telemedicine.
• Complete regulatory visits and participate in family conferences.
• Address pharmacy and medication recommendations.
• Be present at assigned facilities during work hours to ensure adequate face-to-face coverage.

Qualifications & Experience

Required:

• Master of Science in Nursing, DNP, or NP certification.
• Certification from AANP or ANCC.
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• At least 2 years of clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and environment.
• Passion for senior and low-income healthcare.

Preferred:

• Geriatrics training or experience.
• Over 3 years of NP clinical experience.
• Experience with coding, ICD-10, HCC.
• Experience in results-driven healthcare models.
• Willingness for professional development.
• Experience in value-based care, outpatient primary care, or assisted living settings.
