Job summary

Carilion Clinic in Lexington is seeking a Home Health Aide for a part-time role. The aide will provide care and comfort measures to patients in their homes under the care plan and interdisciplinary plan of care.

The position requires a high school diploma or GED, CMS-compliant Home Health Aide certification, a valid driver's license, and prior aide experience or completion of a recognized program. Benefits are offered with compensation detailed below.

Qualifications

• Education: H.S diploma or GED
• Experience: Must have worked as an aide within the 24 months prior to hire or have completed another CMS-defined program
• Licensure and/or certification: Certified as a nurse's aide and VA registration; meets CMS requirements for Home Health Aide training; valid driver's license

Responsibilities

• Provides care under direction of the clinical leader
• Reviews care plan and records visit notes; documents care and findings
• Reports concerns or changes to leadership
• Assists with activities of daily living and maintaining home comfort
• Follows the care plan at all times
• Provides basic personal care and hygiene assistance
• Delivers bedside care per Home Care/Hospice policies and aide plan
• Documents oral/ topical medications patient can self-administer
• Monitors fluid intake/output as indicated
