Job summary

UC San Diego Health is seeking an RN Care Manager to coordinate patient care from pre-admission through discharge. You will work with physicians, nurses and social workers to facilitate medically appropriate, cost-effective care and ensure smooth patient transitions.

The role requires CA RN licensure, strong communication, and experience with InterQual and discharge planning in a fast-paced hospital setting; familiarity with Epic/ECIN is a plus.

Qualifications

• RN license issued by the State of California
• Bachelor of Science in Nursing (BSN) or higher preferred
• ACM and/or CCM certification preferred
• ADN RN with minimum 3 years acute care manager experience
• Minimum 3 years inpatient RN care manager experience in acute care hospital within last 7 years
• Two years of experience with InterQual
• Experience with discharge planning and utilization review
• Proficient in MS Office (Word, Excel, Outlook, PowerPoint) and EMR documentation
