Responsibilities

• Conduct comprehensive and ongoing physical and psychosocial assessments.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Implement care interventions and ensure safe delivery.
• Evaluate effectiveness of interventions and revise care plans based on reassessments.
