Job summary

Monadnock Community Hospital in Peterborough, NH, seeks an Occupational Therapist to join our outpatient lymphedema management team. This part-time role (22 hours per week) focuses on helping patients regain independence and improve quality of life.

You will conduct assessments, develop individualized plans, and collaborate with physicians, nurses, PTs, and social workers. Lymphedema certification or willingness to pursue training is required.

Qualifications

• Lymphedema Management Certification or willingness to actively seek training required.
• BLS certification is required.
• Outpatient setting experience preferred; new graduates welcome to apply.

Responsibilities

• Conduct thorough assessments of physical, cognitive, and emotional abilities and functional limitations.
• Collaborate with patients, families, and healthcare team to set goals and develop individualized treatment plans.
• Provide evidence-based occupational therapy interventions to address impairments and promote independence.
• Utilize therapeutic modalities, exercises, adaptive equipment, and assistive technology to improve mobility, dexterity, strength, coordination, and cognitive skills.
• Offer guidance on compensatory strategies, energy conservation, and home modifications for independent living.
• Monitor progress, track outcomes, and modify treatment plans as needed.
• Maintain accurate documentation of evaluations, sessions, progress, and discharge summaries.
• Collaborate with physicians, nurses, PTs, and social workers for coordinated care.
• Stay current with OT research and best practices to provide high-quality care.
• Other duties as assigned.
