Job summary

Crest Home Health in San Antonio, TX is seeking an LVN/LPN to provide direct patient care, administer prescribed medications, and perform treatments. The role reports to the Case Manager/Patient Care Manager and requires compassion and reliability. Home health experience is preferred, with a be flexible schedule.

Requirements include graduation from a practical nursing program and an active Texas LVN/LPN license with CPR; BLS may be preferred. Prior home health exposure is advantageous.

Responsibilities

• Assists in collecting nursing observations and reporting signs and symptoms to the Case Manager.
• Assists in assessing the health status of assigned patients.

• Uses the written plan of care to carry out nursing interventions.
• Adapts to changes in workload, emergencies, scheduling, or staffing.
• Must have reliable transportation with valid driver's license and proof of insurance.

Job Description

We are looking for a reliable LVN/LPN (Licensed Practical Nurse/Licensed Vocational) to administer direct patient care, administer approved medications, perform treatments, make accurate nursing observation, and chart on the patients' medical record in a professional and considerate manner. Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost qualified and compassionate. The ideal candidate preference will have at least one year experience as a nurse in a health care setting. Home health experience preferred. We offer competitive pay and flexible schedule.

Educational Requirements

Graduate of an approved program of vocational/practical nursing

Licensing/Certification/Registration Requirements

• Current Licensed Vocational/Practical Nurse (LVN/LPN) from the Texas Vocational Nurse Examiners
• Current CPR
• BLS certification preferred.

• Assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager
• Assists in assessing the health status of assigned patients.
• Complies with the established nursing assessment guidelines/policies.
• Utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals.
• Complies with general performance standards of licensed vocational nurse. Demonstrates flexibility in response to unexpected changes in work volume, emergencies, staffing, or scheduling changes.
• Must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state driver’s license)
