We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated:

Licensed Vocational Nurse or Licensed Practical Nurse

(LVN or LPN) - Home Health

Shifts Available:Full-Time, Part Time & PRN

Service Areas:Woodlands, Montgomery, Humble,Kingwood, Clear Lake, Galveston

Responsibilities:

  • Collaborate with the home healthcare teamto develop and implement individualized care plans.
  • Complete home care nursing visits.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate with healthcare providers and other care team members.
  • Educate patients and their families about health conditions, treatment plans, and self-care techniques.
  • Adhere to all agency policies, procedures, and regulatory requirements related to home health care.

Qualifications:

  • Valid and current LVN or LPN license in the state of Texas or compact state.
  • Minimum of one (1) year of experience in a home health setting.
  • Knowledge of home health regulations and general healthcare compliance.
  • Experience with HCHB EMR. (preferred but not required)
  • Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
  • Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
