Job summary

Gallaher Signature Living is seeking an LVN to oversee resident care services across our luxury California senior living communities. The role emphasizes regulatory compliance, individualized care planning, and close collaboration with the Wellness Director, nurses, and care staff.

You will help maintain safety, dignity, and independence for residents and participate in licensing inspections, while ensuring accurate records and smooth transitions of care across our Alameda-area communities.

Qualifications

• Current LVN license for California is required.
• 2–3 years in assisted living, RCFE, or similar senior living environments preferred.
• Knowledge of Title 22 regulations and compliant care delivery.

Responsibilities

• Coordinate and oversee resident care services in compliance with Title 22.
• Develop, implement, and review Individualized Service Plans (ISPs).
• Monitor residents’ status and report changes to Wellness Director and licensed staff.
• Support independence while safeguarding safety, dignity, and choice in daily living.
• Ensure care services align with Title 22 including admissions, reassessments, and documentation.
• Participate in licensing inspections, audits, and corrective action plans as directed.
• Supervise unlicensed staff within Title 22 scope; provide day-to-day coaching.
• Assist with onboarding, training, and mentoring of care team members.
