Job summary

A national healthcare staffing firm is seeking an experienced LVN Service Coordinator to assess patients' Activities of Daily Living (ADLs) and engage with the aged, blind, and disabled population. This role offers a competitive salary ranging from $19 to $23 per hour, with benefits such as medical, dental, and vision coverage. Regular hours are Monday to Friday from 8:00 AM to 5:00 PM, with the possibility of overtime or weekend shifts. Qualifications

• Approximately 2 years of LTC or Home Health experience required.
• Experience working with SSI patients on disability preferred.
• Must have experience assessing ADLs using the 2060 form.

Job Description

Daily Responsibilities

• Will be completing “2060 Assessments,” assessing patients’ ADLs (Activities of Daily Living) and need for services
• Talking on the phone with the ABD (aged, blind, disabled) population

Hours for this Position

• Mon-Fri 8:00-5:00, possibility of OT and/or Saturdays
