Job Description

Are you an experienced LVN looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes” to any of these questions – this is the LVN position for you!

Daily Responsibilities:

• Telephonic office position
• Assessing patients’ ADLs (Activities of Daily Living) and doing HRAs (health risk assessments) and care plan reviews to assess the need for services that allow members to remain out of the hospital
• Working with the ABD (aged, blind, disabled) population

Hours for this Position:

Monday through Friday 8am-6pm and some Saturdays
