Job summary

HMG Healthcare LLC is seeking an LVN/RN Charge Nurse at Oak View Rehabilitation and Wellness Center in Gilmer, TX. The role focuses on delivering compassionate nursing care, administering medications, monitoring vital signs, and supporting activities of daily living under RN/MD supervision.

Qualifications include Texas LVN/RN licensure and BLS; prior long-term care experience is preferred. The team collaborates closely with multidisciplinary staff to ensure quality patient outcomes and dignity.

Qualifications

• Current and valid LVN or RN license in Texas.
• High school diploma or equivalent.
• Basic Life Support (BLS) certification.
• Demonstrated experience in a long-term care or rehabilitation setting preferred.
• Strong understanding of nursing principles, medication administration, and patient care procedures.

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments as prescribed.
• Monitor and record patients' vital signs, symptoms, and overall health status, reporting any changes to the healthcare team.
• Assist residents with activities of daily living such as bathing, dressing, and mobility support.
• Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
• Maintain accurate and detailed patient records in compliance with healthcare regulations and facility policies.
• Educate residents and their families on health maintenance and disease prevention as appropriate.
• Ensure a safe and clean environment by adhering to infection control protocols and safety standards.

About the Role:

As a LVN/RN at Oak View Rehabilitation and Wellness Center, you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the health and well-being of patients by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. This position requires a strong commitment to patient advocacy, ensuring that each resident’s needs and preferences are respected and met with dignity. Minimum Qualifications:

• Current and valid LVN or RN license in the state of Texas.
• High school diploma or equivalent.
• Basic Life Support (BLS) certification.
• Demonstrated experience in a long-term care or rehabilitation setting preferred.
• Strong understanding of nursing principles, medication administration, and patient care procedures.

Preferred Qualifications:

• Experience working in a skilled nursing or rehabilitation facility.
• Additional certifications such as IV therapy or wound care.
• Familiarity with electronic health record (EHR) systems.
• Excellent communication and interpersonal skills.
