The Meadows of Napa Valley in Napa, California is seeking a Licensed Vocational Nurse (LVN) to provide person-centered care to residents, including medication management and assessments, in compliance with regulatory standards.

This role requires at least 1 year of long-term care nursing, active LPN/LVN license, CPR certification, and proficiency with electronic health records; salary ranges from $32.00 to $40.00 per hour with DOE.

Qualifications

• Minimum of 1 year professional nursing experience, especially in long‑term care.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
• Current CPR certification.
• Demonstrated understanding of functions, organization, policy, regulations, and procedures of a long‑term care or skilled nursing facility.
• Competence in use of electronic health records and other clinical software.

Responsibilities

• Provide person-centered, individualized care to all residents, ensuring services comply with regulatory standards.
• Advocate for resident rights and best interests in operational decision-making.
• Delegate tasks and maintain accountability, communication, and effective teamwork.
• Administer medications and treatments, maintain standards of practice, uphold universal precautions, prepare medication logs, and monitor drug effects.
• Identify, gather information, and report changes in resident conditions; document findings; notify physicians and responsible parties; create resident-specific Service Plans.
