The role emphasizes one-on-one care, adherence to care plans, and collaboration with an interdisciplinary healthcare team. This position offers full-time and part-time schedules with flexible shifts, including days, evenings, and overnights, and provides weekly pay with benefits.

Qualifications

• Active LVN license required.
• Current BLS certification required.
• 1+ year of nursing experience preferred.

Responsibilities

• Provide direct patient care in the home.
• Monitor patient condition and report changes.
• Administer medications and treatments as ordered.
• Support individualized care plans.
• Document care and communicate with the healthcare team.
