Thrive Rehabilitation of Pearland is seeking a compassionate LVN to join our team in Pearland, Texas. You will provide direct patient care in a post-acute rehab setting and collaborate with a multidisciplinary team to achieve patient goals.

Applicants must hold a Texas LVN license, have a high school diploma or equivalent, and be able to assist with transfers and activities of daily living. Join our dedicated care team in a serene facility focused on quality patient outcomes.

Qualifications

• Must have current LVN licensure in Texas.
• High school diploma or equivalent required; associate's degree preferred.
• Ability to lift/move and stand/walk for extended periods.
• Strong communication and collaboration with patients and healthcare team.

Responsabilités

• Provide direct patient care including vital signs, medications, wound care, and equipment training.
• Develop and implement individualized care plans with other healthcare professionals.
• Monitor patient progress and adjust plans as needed.
• Collaborate with rehabilitation professionals to promote independence and goals.
• Maintain accurate records of care, treatment, and progress.
