Job summary

PARADIGM REHAB & HEALTH CARE LLC is looking for a Licensed Vocational Nurse (LVN) to provide direct patient care to home bound patients in the Plano, Richardson, and Sachse area. The role requires strong clinical skills, coordination with interdisciplinary teams, and an advocacy for patient care.

Ideal candidates will possess a current Texas LVN License and CPR certification, with additional experience in home health care being a plus. The position offers various benefits including insurance, a retirement plan, and ongoing education support.

Qualifications

• Must hold a current Texas LVN License.
• Certification in BLS/CPR is required.
• Knowledge of home health care procedures is beneficial.

About this position

LVN will provide direct patient care to home bound patients.

Responsibilities and Duties: Plano, Richardson, Sachse Area

• Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.
• Carries out the clinical plans of care to assist patients maintain their independence.
• Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
• Provides education to patients and families on proper home health care procedures and strategies.
• Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.

Qualifications and Skills

• Current Texas LVN License.
• Current Certification in BLS/CPR.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.
