Job summary

Sweetwater Care is seeking an LVN with leadership experience to oversee nursing operations in our California facility. The role emphasizes timely medication and treatment administration, adherence to physician orders within the Nurse Practice Act, and active rounds with physicians as needed.

The ideal candidate holds an active California LVN license, with preferred background in SNF/LTC settings and experience using Point Click Care (PCC) and paper charting to ensure high-quality patient care

Qualifications

• California LVN license (RN not required) is required.
• 1 year nursing management experience preferred.
• SNF/LTC experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in Quality Improvement Program.
• Assists in patient identification, placement, and monitoring.
• Notifies departments of patient changes and admissions/discharges.
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Processes physician orders as allowed by the Nurse Practice Act.
• Makes rounds with physicians when necessary.
