Job summary

WellMed, part of the Optum family, seeks a Licensed Vocational/Practical Nurse (LVN/LPN) to provide high-quality clinical support and patient care in a Texas clinic. You will work under providers and RNs, delivering nursing services, documentation, and patient education in a team-based setting.

Required credentials include an unrestricted LVN/LPN license, IV certification if needed, and BLS. Strong communication and EMR skills are essential for effective patient interactions and care

Qualifications

• Current and unrestricted State Vocational/Practical Nursing license
• IV certification, if required by clinic
• Current BLS certification
• Proven knowledge of medical terminology and medical office procedures
• Proficient computer skills to work with electronic medical records
• Proven effective listening and communication skills; proper grammar and spelling
• Proven good communication and customer service skills

Responsibilities

• Performs nursing duties within the scope of LVN/LPN (injections, meds, EKGs, phlebotomy) as allowed by state board
• Participates in patient education plans for common health problems
• Coordinates patient care with providers, RNs, and policies
• Documents patient assessments and prepares for examination
• Relays patient information to providers telephonically
• Performs focused nursing assessments and communicates findings
• Records data in the medical record accurately
• Reviews patient records and lab/x-ray results prior to visits
• Maintains a safe environment and patient confidentiality
• Organizes and sterilizes exam rooms and equipment
• Follows infection control precautions and medication documentation
• IV therapy per clinic needs (if certified)
• Performs other related duties as assigned
