Job summary

Sierra Valley Rehab Center in Porterville, CA is seeking Licensed Vocational Nurses (LVNs) to join our skilled nursing team. The LVN will provide direct nursing care under supervision, administer medications, supervise CNAs, and ensure compliance with facility standards.

Qualifications

• Current California LVN license in good standing.
• Graduate of an accredited LVN program.
• CPR certification required.
• Prior long-term care experience preferred.
• Excellent communication, critical thinking, and leadership skills.
• Proficient in basic computer use and electronic medical records (PCC experience preferred).

Responsibilities

• Supervise and support CNAs in delivering quality care.
• Administer medications and treatments as prescribed.
• Chart all care provided, resident responses, and any changes in condition.
• Assist with admissions, discharges, and transfers.
• Participate in care planning, quality assurance initiatives, and staff development.
