Gateway Post Acute in Porterville, CA is seeking a Licensed Vocational Nurse (LVN) for a part-time role. The LVN will provide direct nursing care, follow care plans, administer medications, and work with RNs, CNAs, and the interdisciplinary team to support resident well-being.

Shifts available AM/PM/NOC; compensation is $28-$30 per hour (BOE). This position offers a supportive team environment and opportunities for ongoing training and development.

Qualifications

• Graduation from an accredited Vocational Nursing program
• Current, active California LVN license in good standing (CA LVN)
• Current BLS/CPR certification
• Experience in skilled nursing facility or long-term care preferred
• Strong clinical and assessment skills
• Excellent communication and documentation abilities

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders
• Administer medications and treatments accurately and timely
• Monitor residents for changes in condition and report findings promptly
• Document care provided, observations, and resident responses in the medical record
• Assist with admissions, transfers, and discharges as needed
• Collaborate with RNs, CNAs, and the interdisciplinary team
• Maintain infection control, safety, and privacy standards
• Respond to resident needs and emergencies promptly
