Qualifications

• Current, active California LVN license.
• CPR/BLS certification required.
• Minimum 1 year of experience in skilled nursing or post-acute care preferred.

Responsibilities

• Provide direct nursing care in accordance with established care plans and nursing standards
• Administer medications and treatments as prescribed by physicians
• Monitor, assess, and document resident conditions, reporting changes promptly to the RN or physician
• Assist with resident admissions, discharges, and transfers
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Promote a safe, clean, and supportive environment for residents, visitors, and staff
• Deliver compassionate, respectful care while maintaining resident dignity and independence
