Job description

Job Summary:

Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assists medical staff to ensure optimal care within established policies, procedures, and standards. Performs nursing duties in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Demonstrates commitment to the organization's Mission, Vision, and Strategic Plan. Maintains high-quality work habits and clinical practices, treating coworkers, patients, families, and all members of the healthcare team with dignity and respect.
Essential Responsibilities:

  • Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; adheres to critical service standards.
  • Provides therapeutic patient care as established by physicians, advanced practice providers, or registered nurses within scope of practice.
  • Transcribes physician/advanced practice provider orders following policy.
  • Communicates with registered nurses to modify the care plan; assists in developing nursing care plans.
  • Administers medications accurately according to provider orders and within scope of practice.
  • Performs treatments such as urinary catheterizations, IV placements, irrigations, suctioning, throat irrigations, enemas, and uses related equipment, adhering to infection control policies.
  • Follows high standards of disinfection, including point-of-use and peel packs policies.
  • Takes vital signs, performs wound care, instructs patients on self-administered treatments, and collects specimens ensuring proper procedures.
  • Assists medical staff during examinations and treatments, providing support and restraint as needed.
  • Observes and documents patient conditions, initiates emergency procedures if necessary, and reports findings promptly.
  • May start and maintain IV therapy and perform venipuncture with appropriate certifications, notifying staff of any issues.
  • Collects and processes specimens per protocols.
  • Provides basic patient education following care plans, reporting learning needs and discharge barriers.
  • Ensures cleanliness and proper functioning of nursing stations and equipment, maintaining a safe environment.
  • Uses effective communication skills.
  • Performs additional departmental duties including back-office workflows, patient outreach, and care gap closure.
  • Participates in staff meetings and training programs.
  • Assists with supply management and accepts orders from physicians and providers.
  • Supports patient communication via email, calls, and messages.
  • Administers oxygen therapy, suctioning, and sputum induction, documenting all care appropriately.
  • Performs other duties as assigned, including orientation of new staff and discharge coordination.
  • Functions within license and scope of practice, promoting excellent customer service and compliance with policies and legal requirements.
  • Utilizes skills assessments to ensure proficiency across patient age groups.
Basic Qualifications:

  • Minimum 12 months of LVN direct patient care experience within the last three years, including medical office, acute care, home care, or skilled nursing.
Education:

  • High School Diploma or GED.
License, Certification, Registration:

  • Vocational Nurse License (California).
  • Basic Life Support certification.
Additional Requirements:

  • Strong customer service skills and dedication to quality care.
  • IV and Blood Withdrawal Certifications preferred.
