Job Summary
Provides nursing care under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assist medical staff to ensure optimal patient care within established policies, procedures, and standards. Demonstrates commitment to the organization’s Mission, Vision, and Strategic Plan, and treats all team members, patients, and families with dignity and respect.
Essential Responsibilities
- Exhibit excellent customer relations, showing courtesy, compassion, and respect to patients, visitors, providers, and coworkers.
- Provide therapeutic patient care as directed by physicians or registered nurses within scope of practice.
- Transcribe orders from physicians or providers, following policy.
- Communicate with registered nurses to modify care plans and assist in their development.
- Administer medications accurately according to provider orders and within scope of practice.
- Perform treatments such as catheterizations, IV placements, irrigations, suctioning, and other procedures, adhering to infection control policies.
- Monitor and document patient conditions, report significant findings, and assist in emergency procedures as needed.
- Support IV therapy and blood withdrawal with appropriate certifications, notifying staff of any issues.
- Collect and process specimens per protocols.
- Provide patient education following care plans, reporting any barriers or needs.
- Ensure cleanliness and proper functioning of equipment and environment.
- Support departmental workflows including patient outreach and care gap closure.
- Participate in staff meetings and training programs.
- Assist with ordering and stocking supplies.
- Manage patient communication and messages effectively.
- Document care activities in Health Connect as per policies.
- Assist in discharge coordination and orientation of new staff.
- Perform all duties within licensure and scope of practice, promoting excellent customer service and compliance with policies and laws.
- Utilize skills assessments to ensure proficiency across age-related patient needs.
Experience & Education
- Minimum 12 months of LVN direct patient care experience in the last three years.
- High School Diploma or GED.
- California Vocational Nurse License and BLS certification.
Additional Requirements
- Strong customer service skills and dedication to high-quality care.
- Bilingual (English/Spanish) at QBS Level II required.
Preferred Qualifications
- IV and Blood Withdrawal Certifications preferred.
Notes
- Must pass bilingual test within the last 12 months or be active in the QBS program.
- Schedule includes rotating weekends, holidays, and floating holidays based on seniority and service line.
