Job Summary:
Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assists medical staff to ensure optimal care within established policies, procedures, and standards. Performs nursing duties in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Demonstrates alignment with the organization's Mission, Vision, and Strategic Plan, and maintains quality and effectiveness in work habits and clinical practice, treating coworkers, patients, families, and all members of the healthcare team with dignity and respect.
Essential Responsibilities:
- Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; adheres to department service standards.
- Provides therapeutic patient care as directed by physicians/advanced practice providers or registered nurses within the scope of practice.
- Transcribes physician/advanced practice provider orders following policy.
- Communicates with registered nurses to modify the care plan; assists in developing the nursing care plan.
- Administers medications accurately following provider orders or standing orders within scope of practice.
- Performs treatments such as urinary catheterizations, IV placements, irrigations, suctioning, throat irrigations, enemas, and uses equipment as required, adhering to infection control policies.
- Follows high-level disinfectant, point-of-use, and peel pack policies and procedures.
- Performs patient care functions such as vital signs, wound care, patient education, specimen collection, and culture collection.
- Assists medical staff during examinations and treatments by passing instruments, supporting, restraining, and draping patients as needed.
- Observes and documents patient conditions, identifies significant findings, initiates emergency procedures if necessary, and reports unusual conditions promptly.
- May volunteer to start and maintain IV therapy with certifications, and notify staff of any issues. May discontinue IV therapy except intra-caths and central lines.
- May perform venipuncture/blood withdrawal with certifications.
- Collects and processes specimens per policies and protocols.
- Provides non-complicated patient education following care plans, reports learning needs and discharge barriers.
- Ensures cleanliness of the nursing station, patient rooms, and proper functioning of equipment to maintain a safe environment.
- Uses effective verbal and non-verbal communication.
- Performs duties in other departments or modules as assigned to maintain standards of care and safety.
- Participates in departmental workflows including patient outreach, closing care gaps, and nurse visits.
- Attends staff meetings and in-service programs.
- Assists with ordering and stocking supplies and equipment.
- Supports patient communication and message management, including emails and call center messages.
- Administers pulse oximetry, humidification therapy, spirometry, oxygen therapy, suctioning, and sputum induction.
- Documents care in Health Connect as per policies and protocols.
- Performs other duties as assigned, including orientation of new employees.
- Assists in coordinating discharge processes.
- Performs all functions within license and scope of practice.
- Promotes excellent customer service and maintains confidentiality and compliance with legal and organizational standards.
- Ensures proficiency in age-related competencies for patient populations served.
Basic Qualifications:
- Experience: Minimum twelve (12) months of LVN direct patient care experience within the last three (3) years, including medical office, acute care, home care, or skilled nursing facility.
- Education: High School Diploma or GED.
