What You’ll Be Doing:

Care Coordination & Support – Assist with patient records, care plan updates, and survey prep

️ Patient & Provider Communication – Follow up on orders, coordinate with doctors, and help patients get the services they need

️ Compliance & Quality Assurance – Help ensure documentation is accurate and that we’re always ready for audits or visits

Jump In Where Needed – Support intake, handle clinical questions, assist the administrator or DON — be the reliable go-to

Who We’re Looking For:

Licensed LVN or RN in Texas

1–3+ years of nursing experience (home health experience preferred but not required)

Independent & Adaptable – You’re comfortable working without constant supervision and love variety

Excellent Communicator – You work well with patients, doctors, and teammates

Detail-Oriented – You’re great at spotting what’s missing and making sure nothing falls through the cracks

Spanish-speaking is a plus, but not required
