Job summary

Sierra Valley Rehab Center in Porterville, CA is seeking a part-time Licensed Vocational Nurse (LVN) to join our skilled nursing team. Administer medications, supervise CNAs, monitor resident care, and ensure regulatory compliance.

Qualifications

• Current and unencumbered LVN license in California.
• CPR certification required.
• Experience in long-term care or skilled nursing is preferred.

• Proficient with basic computer use and electronic medical records (PCC experience preferred).

Responsibilities

• Supervise and support CNAs in delivering quality care.
• Administer medications and treatments as prescribed.
• Chart all care provided and resident changes.
• Assist with admissions, discharges, and transfers.
• Participate in care planning, QA initiatives, and staff development.
• Conduct rounds and ensure compliance with nursing procedures.
• Respond to emergencies with appropriate interventions.
• Collaborate with interdisciplinary teams and families.
• Maintain accurate and timely documentation per policies.
• Promote resident rights, dignity, and confidentiality.
