Job summary

Kern River Transitional Care Center is seeking an LVN for the Full-Time NOC shift to provide compassionate resident care and collaborate with the interdisciplinary team in Bakersfield, CA.

The ideal candidate holds a California LVN license with CPR/BLS, and has experience in skilled nursing or post-acute settings. In addition to competitive pay, a $1.00/hour NOC shift differential is offered along with comprehensive benefits.

Responsibilities

• Provide direct nursing care to residents in accordance with physician orders and care plans.
• Administer medications and treatments accurately and timely.
• Monitor residents for changes and report concerns to physician or nursing supervisor.
• Complete accurate and timely nursing documentation.
• Participate in development and implementation of individualized care plans.
• Communicate effectively with residents, families, physicians, staff and interdisciplinary team.
• Assist with admissions, transfers, and discharges as needed.
• Obtain and document vital signs and clinical information.
• Ensure physician orders are carried out accurately.
• Supervise and provide direction to CNAs as assigned.
• Follow infection prevention and control procedures.
• Respond to emergencies and changes in condition.
• Maintain confidentiality of resident and facility information.
• Ensure residents are treated with dignity, respect, and compassion.
• Follow all nursing regulations, policies, and procedures.
• Perform additional nursing duties as assigned.

Licensed Vocational Nurse (LVN) – Full-Time NOC ShiftKern River Transitional Care Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the Full-Time NOC Shift.
