Your Role

• Provide direct patient care and case management in the home
• Conduct patient admissions and develop individualized care plans
• Obtain medical history and perform brief physical exams (vitals, mental status, diet, etc.)
• Deliver skilled nursing care in compliance with HIPAA
• Evaluate effectiveness of nursing services and coordinate total patient care
• Submit patient notes and documentation in a timely manner
