Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of operation.
• One year of LVN/LPN work experience.
• Ability to provide nursing visits and maintain medical records.
• Must have CPR certification.
• Must be a licensed driver able to travel to all locations.

Responsibilities

• Performs direct patient care under RN supervision.
• Promotes and maintains optimal client care under RN supervision.
• Maintains a safe patient environment.
• Communicates significant findings to the case manager and home health team.
• Administers prescribed medications and evaluates drug therapies for safety.
• Re-evaluates nursing needs and reports findings to the Case Manager.
• Counsels and instructs clients and families on nursing needs.
• Prepares progress notes and documentation timely.
