Job summary

MedCadre in the United States (Oregon/Portland area) is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled home health care for geriatric patients. Ideal candidates have recent home health experience, are proficient with HCHB, and can manage patient visits efficiently with accurate documentation.

You will collaborate with the healthcare team, maintain care plans, and support a substantial daily caseload while traveling within the territory.

Qualifications

• Active LPN license in good standing.
• At least 1 year of recent home health experience.
• Experience with Home Care Home Base (HCHB) is required.
• Current BLS certification.
• Ability to meet the weekly productivity of 30 points.
• Reliable transportation and ability to travel within the assigned territory.
• Strong documentation and time-management skills.
• First-time travelers are not accepted.

Responsibilities

• Complete patient visits, monitor conditions, report changes as needed.
