Job summary

Excelin Home Health is seeking a Licensed Practical/Vocational Nurse (LVN/LPN) for the Grass Valley service area. The PRN position supports direct patient care under the supervision of a registered nurse and collaborates with the care team to implement the plan of care.

The LVN/LPN will document care, educate patients and families, and coordinate with other disciplines to ensure quality home health services. Current California LVN license and CPR certification are required.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience with one year in community/home health or medical/surgical experience.
• Currently licensed as an LPN or LVN in the state.
• Complies with professional standards and practice.
• Demonstrates good verbal and written communication and organization skills.
• Must be a licensed driver with insured automobile.
• Possesses and maintains CPR certification.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Implement plan of care initiated by the RN and document accordingly.
• Coordinate with other disciplines and report needs to the RN/Clinical Director.
• Educate patients and families per diagnosis and plan of care.
• Maintain proper use of equipment and supplies; participate in growth and development.
• Prepare clinical notes and communicate with the office team professionally.
• Notify manager/RN of any patient care concerns or issues.
• Perform other duties as assigned by the RN.

Position Summary: The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Provides direct patient care as defined in State’s Name Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager/Clinical Director.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Communication

• Prepares clinical notes and updates the registered nurse when necessary.
• Communicates effectively and works closely with the office team in a professional manner and ensures they are aware of any scheduling or patient difficulties.
• Informs manager and/or RN of any patient care concerns or issues.

POSITION QUALIFICATIONS

1. Graduate of an accredited practical nurse or vocational nursing program.

2. Has three years nursing experience and, effective January 1, 1998, shall have one year prior professional nursing experience in Community/home health or medical/surgical experience.

3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.

4. Complies with accepted professional standards and practice.

5. Demonstrates good verbal and written communication, and organization skills.

6. Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements.

7. Possesses and maintains current CPR certification.

SKILLS REQUIRED
