Job summary

Excelin Home Health is seeking a Licensed Practical/Vocational Nurse (LVN/LPN) for PRN clinical nursing in Pflugerville/Austin, TX. The role provides direct patient care under RN supervision and requires collaboration with the Clinical Manager/Clinical Director.

Qualified candidates will have a state-licensed LPN/LVN, CPR certification, and at least three years of nursing experience including community/home health or medical/surgical exposure. A valid driver’s license is required.

Position Summary: The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under

the supervision of a registered nurse. Responsibilities include following the plan of care,

providing treatments, and working collaboratively with the members of the team to help meet

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Provides direct patient care as defined in State’s Name Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to
• diagnosis and plan of care
• Participates in coordination of home health services, appropriately reporting the identified needs for
• other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical

Communication

• Prepares clinical notes and updates the registered nurse when necessary.
• Communicates effectively and works closely with the office is team in a professional manner and

• Informs manager and/or RN of any patient care concerns or issues.

POSITION QUALIFICATIONS

1. Graduate of an accredited practical nurse or vocational nursing program.

experience in Community/home health or medical/surgical experience.
