Qualifications

Responsibilities

• Support quality improvement initiatives and care coordination for high-risk patients.
• Identify care gaps by pulling reports and participate in QI programs.
• Maintain communication with providers via reporting tools and meetings.
• Assist patients in scheduling preventive screenings and follow-up care.
• Provide health education and counseling using standardized materials.
• Serve as a resource on care transitions and quality metrics.
• Demonstrate leadership in a high-performance clinical environment.
