Job summary

Sweetwater Care in Willows, CA is seeking an LVN/LPN to provide nursing care within scope of practice and in accordance with physician orders and center policies. The role supports quality patient care under state guidelines.

Minimum qualifications include a California LVN/LPN license, LTC experience, and BLS/CPR certification. Experience in nursing management is preferred but not required. Willingness to obtain certifications is a plus.

Qualifications

• Licensed LVN/LPN in the state of practice.
• Long Term Care experience is required.
• BLS/CPR certification or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Manages narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, skin, weight, etc.).
• Uses Quality Life Manual for patient identification and program monitoring.
• Documents and notifies departments of room changes, LOA, new admits or discharges.
• Rounds with physicians when necessary.
