Job summary

Valley View Skilled Nursing and Rehab is seeking a Licensed Vocational Nurse (LVN) to deliver efficient nursing care, mentor staff, and coordinate resident care in a busy long-term care facility. You will work under state regulations and collaborate with physicians to ensure high-quality outcomes and family satisfaction.

The role offers full-time, part-time, and PRN shifts with a competitive pay range and a $2,500 sign-on bonus for qualified full-time candidates.

Qualifications

• Valid state license(s).
• Excellent verbal communication and strong interpersonal skills.

• At least one year’s experience in long-term care (preferred).

Responsibilities

• Conduct the daily nursing functions in accordance with Company, State, Federal and local rules, regulations, and guidelines.
• Admit, transfer and discharge residents with documentation regarding care and service provided.
• Participate in the orientation of new residents/family members to the facility.
• Participate in facility surveys (inspections) as scheduled.
• Ensure delivery of high-quality, positive clinical outcomes, and resident/family satisfaction during shift.
• Serve as a resource to all staff regarding the quality of services provided.
• Chart progress notes in an informative, factual manner reflecting care and resident responses.
• Follow procedures for charting and reporting all incidents/accidents.
• Identify and report changes in condition to supervisor, physician and family.
• Identify skin changes and follow skin management protocols.
• Order and report diagnostic tests timely.
• Count narcotics at shift start and end; follow policies for medication delivery.
• Follow procedures for restraint reduction.
