Job summary

Bristol Hospice is seeking a compassionate Licensed Vocational Nurse (LVN) to join our Modesto team. This PRN, hourly position covers weekend graveyard shifts and offers pay of $35.00-$37.00 per hour.

Provide direct patient care in end-of-life hospice settings, document care plans, and work closely with an interdisciplinary team to support patients and families. California LVN license required; hospice experience preferred.

Qualifications

• Active LPN/LVN license in California required.
• Minimum 3 years nursing experience; hospice or medical/surgical care preferred.
• Understanding of hospice philosophy and terminal patient needs.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act, including end-of-life duties.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Educate patient and family/caregiver about diagnosis and self-plan of care.
• Coordinate hospice services with the interdisciplinary team and report needs.
• Use and prepare equipment adhering to aseptic technique.
• Provide pain and symptom management and assess patient response to treatments.
• Participate in ongoing professional development and quality improvement activities.

QualificationsAn Average Day:

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
