Job summary

Talented Medical Solutions is seeking an LVN for Home Health/Hospice in Sonora, CA. This 13-week contract requires 40 hours/week, with day shifts and weekend coverage. Start date around 08/31. Prior hospice experience preferred.

Role involves nursing procedures, plan-of-care development, on-call duties, and patient education under supervision of an RN/MD. Requires LVN licensure in CA and BLS; driver’s license preferred.

Qualifications

• High School Education/GED or equivalent: Preferred
• Bachelor's Degree or equivalent combination of education/related experience: Preferred
• Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
• Basic Life Support (BLS) Health Care Provider certification: Required
• Arrhythmia certification: Preferred
• Valid Driver's License: Preferred

Responsibilities

• Provides care for assigned patients under the supervision of a Registered Nurse (RN).
• Performs routine visits based on policy and procedures.
• Assists in development of a prioritized interdisciplinary plan of care.
• Implements and coordinates plan of care and teaching.
• Performs clinical procedures including phlebotomy and injections and administers medications.
• Orders/restocks supplies as assigned and maintains area cleanliness.
• On-call duties may be assigned.
