Avir Trilogy at French Home in San Antonio, TX is seeking a compassionate Licensed Vocational Nurse (LVN) to join our care team. You will provide direct patient care to residents and support daily clinical operations.

This role requires Texas LVN licensure and at least 1 year in Long Term Care. You will work with an interdisciplinary team, document care, and participate in quality initiatives to enhance resident well-being.

Qualifications

• Current LVN licensure in the state of Texas.
• 1+ years of experience as an LVN in Long Term Care.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Strong clinical skills and knowledge of geriatric care.

Responsabilités

• Provide direct patient care to residents in accordance with established policies and procedures.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Monitor and report changes in resident condition, including vital signs and laboratory results.
• Implement and update care plans in collaboration with the interdisciplinary care team.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
• Maintain accurate and detailed records of patient care and treatment.
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordinated patient care.
