Job summary

Harbor Healthcare System is seeking a full-time Quality Assurance LPN/LVN in the Houston area. The role focuses on quality control of health services and compliance reviews of documentation to ensure appropriate care.

You will review nurses’ notes, admission packets, and care plans to ensure accuracy and regulatory alignment. Qualified candidates hold an associate degree, a current LPN/LVN license, and at least one year of hospice/home health experience.

Qualifications

• Associate degree from a two-year college (A.A.) or equivalent.
• At least one year of hospice/home health or related field experience.
• Current State LPN/LVN license.
• Effective written and verbal communication.
• Ability to coordinate patient care with other disciplines.
• Valid driver's license.

Responsibilities

• Ensure quality control of health services and review documentation for care appropriateness and compliance.
• Review nurses’ notes, admissions packets, POC, and re-certifications for accuracy and compliance.

We are currently seeking qualified candidates for a full time Quality Assurance (QA) LVNNurse positionin the Houstonarea.

Job Description

A Quality Assurance LPN/LVN is responsible for assuring quality control of all health services provided by the Agency and for reviewing documentation for compliance and appropriateness of care.

Qualifications

• Must have an associate degree (A.A.), or equivalent from a two (2) year college.
• Must have at least one year’s experience in hospice, home health or a related field.
• Must have a current State LPN or LVN license.
• Must be able to speak and write effectively.
• Must work well with other disciplines to coordinate patient care.
• Must be able to prepare forms for pre and post payment audits.
• Must have a current State driver's license
• Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, regulations required
• Experience in reviewing admission packets required
• Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan
• Conducts one-on-one in-services for documentation problems as needed
• Effective written and verbal communication
• Ability to travel - must have reliable transportation
• Clean background and drug screen
