Job summary

A HUG AWAY, INC is seeking a Licensed Vocational Nurse (LVN) to administer skilled nursing care in patients' homes and coordinate care with the hospice and home health interdisciplinary team in the Katy/Houston area.

Under RN supervision, you will support the plan of care, assist with end‑of‑life care as needed, manage supplies, and educate patients and families on self‑care and safety, while communicating clear updates to the office and field staff.

Qualifications

• Graduate of an accredited LVN program.
• Active Texas LVN license and a valid driver’s license.
• Two years LVN experience in clinical, hospice or home health preferred.
• Strong interpersonal skills and ability to follow care plans.
• Spanish language ability preferred but not required.

• Under the direction of the RN, supplement nursing care needs as specified in the plan of care.
• Assist the RN in carrying out the plan of care.
• Help the patient learn appropriate self‑care techniques.
• Provide services in accordance with organizational policies, procedures, and regulatory scope of practice.
• Assist with specialized duties related to end‑of‑life care.
• Prepare equipment and materials for treatment, adhering to aseptic technique.
• Assess patient/family response to care and communicate information to appropriate team members.
• Promote agency philosophy and administrative policies to ensure quality of care.

Job Description

The primary function is to administer skilled nursing care and coordinate care for patients of all ages. The nurse acts as liaison for the field and office, supporting the hospice and home health interdisciplinary team, patients and families, and the agency.

Job Qualifications

• Education: Graduate of an accredited school of vocational nursing.
• Licensure: Current state license as a Licensed Vocational Nurse and a current state driver’s license.
• Experience: Two years as an LVN in a clinical setting, hospice, or home health (preferred).
• Skills: Nursing skills according to generally accepted standards of practice and good interpersonal skills.
• Transportation: Reliable transportation with valid auto liability insurance.
• Spanish speaker preferred but not required.
• Wound care, infusion, vent, and trach experience preferred.
• Experience working with all ages preferred.
• Case management required.

Environmental And Working Conditions

Work takes place in patients’ homes under varying conditions, involving possible exposure to blood, bodily fluids, and infectious diseases. The role requires a flexible schedule, local travel, and occasional work in unpleasant weather. PRN emergency calls may be required.

Physical And Mental Effort

Prolonged standing and walking are required, with the ability to lift up to 50 lb and move patients. The position involves working under stressful conditions to meet patient needs, make quick decisions, acquire resources, and respond to individualized psychosocial needs. Hand‑eye coordination and manual dexterity are essential.

Essential Functions

Work Location

• Job type: fulltime.
• QA and patient coordination – office.
• Follow‑up visits in the Katy/Houston area.

Pay

Hourly rate: $33–35 per hour or $45 per visit, depending on field or office preference and experience.

License/Certification

• Texas LVN License (Required).
