Angel Care Hospice and Palliative is currently seeking an experienced Hospice LVN to provide compassionate end-of-life care for our patients and their families in the Inland Empire area. The ideal candidate lives in and around Bloomington, Colton, Fontana, Highland, Loma Linda, Moreno Valley, Redlands, Riverside, San Bernardino, Yucaipa. Our patients reside in their own residences, assisted living facilities or nursing homes throughout Southern California. The Hospice LVNs communicate directly with patients and caregivers to coordinate their visit schedules. Our nurses work together in a cohesive interdisciplinary team environment to ensure the highest quality of care and services for our patients. We go "above and beyond" to assist our families during a very difficult and emotional time. Our Hospice LVNs are able to rely upon the RN Case Managers and office support staff to assist with processing orders, answering questions and offering guidance.
The ideal candidate is an experienced hospice nurse that is well-versed in using Hospice MD software for documentation. The LVN we are looking for would have excellent communication and time management skills; with an emphasis on superior bedside manners. The key component with this position is schedule flexibility in order to accommodate patient visits on a weekly / biweekly frequencies.
