Job description

Green Meadows Home Health is actively looking for LVNs in the Temecula area to join our growing company and make an impact in the Home Health world.

Position: Licensed Vocational Nurse (LVN)

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with team members to help meet positive patient care outcomes.

Compensation: $50 per visit

Essential Job Functions and Responsibilities
  1. Provide direct patient care as defined in the State Nurse Practice Act.
  2. Implement current nursing practices following comprehensive assessments and the plan of care.
  3. Accurately and timely document patient services to reflect the plan of care.
  4. Assess and educate patients and families/caregivers pertinent to diagnosis and self-care plans.
  5. Coordinate hospice services and report needs to the interdisciplinary team, including hospice aides, OT, PT, MSW, ST, dietitians, or supervisors.
  6. Use equipment and supplies effectively and efficiently.
  7. Manage pain and symptoms appropriately; evaluate patient responses to treatments and medications.
  8. Participate in professional growth, development, and quality improvement activities.
  9. Perform additional duties as assigned by the registered nurse.
Minimum Qualifications
  1. Graduate of an accredited practical or vocational nursing program.
  2. At least three years of nursing experience; community health, hospice, or medical/surgical experience preferred.
  3. Current license as an LPN/LVN in the state.
  4. Compliance with professional standards and practice.
  5. Good verbal and written communication and organizational skills.
  6. Valid driver’s license with insured and well-maintained vehicle.
  7. Current CPR certification.
  8. Understanding of hospice philosophy and terminally ill patient needs.
